*Summary*
The primary role of the Nurse Delegated Coordinator is to provide support to clients on their caseload requiring nurse delegated and other medically intensive tasks.
Assist clients to achieve their goals in accordance with the residential guidelines.
*Essential Duties and Responsibilities* * Acknowledge and advocate for the needs, wants, and rights of clients with dignity and respect.
* Identify areas of change that clients on their caseload would like to achieve.
* Coordinate individualized programs for clients and document activity thoroughly.
* Accompany clients on outings and activities.
* Transport clients to and from all agency activities, and/or arrange transportation services as appropriate.
· Assist with client money management such as petty cash, receipts, checkbook and EBT’s.
· Supervise clients to ensure adequate safety.
* Keep all hazardous materials, medications, etc.
in the designated secured location.
* Implement a safe and clean environment in which our clients live by doing households ADLS (i.
e.
, chores, meals, bathing, hygiene, removal of garbage, laundry, etc.
).
* Report promptly instances of injuries, behaviors, abuse, and safety hazards, etc.
to management, following the Chain of Command.
* Ensure adequate health care for each client.
* Assist clients with mobility needs, transferring them from wheelchair to bed.
* Monitoring vitals (if applicable) and client behavior.
* Turning or adjusting clients in bed to prevent bedsores and/or other discomfort.
* Ensure clients have access to regular meals.
* Accompany clients to health appointments and provide appropriate documentation for each appointment to management in a timely manner.
* Communicate with medical offices, pharmacies, and the nurse delegator.
* Administer medications to clients and document accordingly, including nurse delegated tasks.
* Assist client with daily hygiene as outlined in care plans, including personal care such as brushing teeth, bathing, hair washing, toileting, etc.
* Maintain client confidentially.
* Maintain appropriate staff/client relationships.
* Safeguard all company and client property.
* Maintain an ongoing inventory of client’s personal property.
* Complete all forms, reports, and data sheets for caseload.
* Be aware and abide by all agency policies, procedures, and practices.
· Maintain open communication with supervisor and management team to report any concerns with clients, scheduling, and/or other staff.
* Report to work on time.
* Remain awake and alert while on duty.
* Abstain from partaking of or possessing any illegal drugs or alcohol while on duty.
* Perform other responsibilities as defined by management.
*Education and Experience* * Minimum age of 18 years * Current criminal background check cleared by DSHS before working alone unsupervised with the clients * Valid driver’s license or Washington State ID * Home telephone or cell phone * High school graduate or equivalent * Additional training specific to client complex care and behavioral needs *Skills and Qualifications* · Excellent interpersonal skills, including customer service, patience, empathy and compassion · Effective communication skills, including active listening, writing, speaking and reading comprehension · Good collaboration and teamwork skills · Fast and adaptive problem-solving abilities · Keen attention to detail · In-depth understanding of client care policies and regulations · Familiarity with symptoms and signs of improvement for a number of medical conditions · Comfortable working with specific populations of clients, including elderly clients and those with special needs · Physical strength and stamina · Must be able to successfully pass a background check *Certificates, Licenses and Registrations* · Nursing Assistant Registered (NAR) License, or higher (required) · Home Care Aide (HCA) License (preferred) · Certified Nursing Assistant (NAC) License (preferred) *Knowledge, Skills and Abilities* _Language Skills_: Excellent reading comprehension and written communication skills required.
For example: ability to read, analyze, and interpret documents, reports, technical specifications, governmental regulations, and correspondence.
Ability to apply proper grammar, spelling, punctuation, and format to communications.
Ability to present information in one-on-one, small group, and large group situations to management and staff.
_Mathematical Skills:_ Ability to do simple math such as addition, subtraction, multiplication.
Know how to handle cash and keep a running balance of cash on hand.
_Reasoning Ability:_ Ability to solve a wide range of practical problems and deal with a variety of concrete and complex variables; ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form; and ability to deal with multiple variables.
_Computer Skills:_ Ability to operate related computer applications (i.
e.
word processing and spreadsheet applications), and business equipment including personal computer, copy machine, fax machine, and telephone.
*Physical Demands* _Talking:_ Able to convey detailed or important instructions or ideas accurately, loudly, or quickly.
_Average Hearing:_ Able to hear average or normal conversations and receive ordinary information.
_Finger Dexterity:_ Using primarily just the fingers to make small movements such as typing, picking up small objects, or pinching fingers together.
_Repetitive Motion:_ Movements frequently and regularly required using the wrists, hands, and/or fingers.
_Average Visual Abilities:_ Average, ordinary visual acuity necessary to prepare or inspect documents or products or operate machinery.
_Physical Strength:_ Frequently required to sit, stand, and walk and occasionally required to stoop, kneel, crouch, or crawl.
Occasionally required to lift and/or move up to 50 lbs.
*Your Work Place* Work is normally performed in a home setting, where exposure to conditions of extreme heat/cold, poor ventilation, and fumes are very limited.
Noise level is moderate and may include sounds of normal household equipment (furnaces, telephones, etc.
).
No known environmental hazards are encountered in the normal performance of job duties.
